Labels can be organized by zip code or alphabetically. Avery 5160 labels are used.
To order mailing labels:
If you are paying by credit card, complete and submit the online Mailing Label Order Form. Your order will be processed within 48 hours.
OR
If you are paying by check, complete and print the Mailing Label Order Form (PDF 86K). Information can be typed directly into each field; however, you will not be able to save the document. Navigate by using the tab key. Mail this form, with payment, to:
