These referrals are NOT computer generated. Occasionally, additional information may be needed. In that event, you would receive an e-mail from the Coordinator requesting further details. Please do not include sensitive or confidential information. We consider your information confidential and privileged, but it may be subject to disclosure.

3. You will receive an e-mail confirming that we have received your request and are reviewing your information.
4. A representative from our office will make a referral during our hours of operation and we will contact you via e-mail with this information in one or two business days.
